We offer a Full Service Partnership (FSP) program for young adults ages 16-25 that provides mental health services and supports young people’s independence. Treatment includes involving Transition Age Youth (TAY) in their own futures planning related to employment, education, living situations, and more. The youth learn skills like budgeting and meal preparation, and most importantly, they will also develop their own personal toolkit with coping techniques.

TAY Hot Spot

The center also includes a TAY Hot Spot where young people can come for vocational support, educational support, parenting skills workshops, housing referrals, mental health services, and more. In addition to a staff of licensed and experienced professionals, the Hot Spot employs peer mentors who are dedicated to helping other young people become confident and independent.

Youth Help Network (YHN)

Through community outreach, Youth Help Network teams provide services and resources to Transition Age Youth (TAY) ages 16 through 25 who are experiencing or at risk of experiencing a mental crisis. Youth Help Network connects Sacramento’s youth to local resources for help and inspiration. Teams are located throughout Sacramento County at various community sites for ease of accessibility. A website at www.starsyouth.net features nearly 250 local resources, tools, videos, and helpful information for youth ages 16 through 25. Social media platforms also offer a safe, approachable space where youth can reach YHN staff.

The STAY, a short-term 15-bed Crisis Residential Program, will soon be coming to our center on Marconi Avenue in Sacramento. It will serve TAY ages 18-29 in a home-like environment to help them transition back into community living after a period of psychiatric crisis and recovery. The Flexible Integrated Treatment (FIT) Program

The Flexible Integrated Treatment (FIT) Program provides services and support to children and youth up to age 21 and their families. The FIT team works with everyone involved in each youth’s well-being, such as their family, school, social workers, probation officers, mentors, and others. Treatment focuses on the family, giving individuals a high level of decision-making power. This includes involving teens in planning their own futures related to employment, education, living situations, and more.

Parent-Child Interaction Therapy (PCIT)

Parent-Child Interaction Therapy (PCIT) is an evidence-based behavioral parent-training program focusing on strengthening the parent-child relationship by providing on-the-spot parental coaching. The parent/caregiver plays and interacts with their child in a play room where the clinician can observe the play through a one-way mirror. The trained clinician is in an adjoining room with a mic and earpiece, and the parent/caregiver also wears an ear piece so they can communicate. The clinician provides live parent coaching via telehealth equipment to help parents/caregivers learn and practice child-management skills and increase positive and supportive communication with their child

Programs for children, adolescents, and transitional age youth in Alameda

Stars Community Services provides programs for children, adolescents, and transitional age youth (TAY) who face emotional, behavioral, and learning challenges and having difficulty succeeding in family or community living environments. Based in San Leandro, this dedicated and experienced staff of psychiatrists, psychologists, social workers, mental health specialists, parent partners, and other counselors provide case management, mental health care, and a range of support services in schools, homes or other community settings throughout Alameda County. Our dedicated staff help youth and adolescents receive individualized service planning in the least restrictive setting.

We are still here for you. We are providing services, but in response to the COVID-19 outbreak, we are transitioning to telehealth services over the phone or video for some services. Please call rather than coming to our offices.

Programs & services

Transitional Age Youth Program (TAYP)
An award-winning, outcomes-backed program for youth (ages 18-25), Stars TAY Program is located in San Leandro. Based in research-based clinical practices, these targeted therapeutic, case management and community support services help young adults remain in community living, occupational, and educational settings. The program works with those who have severe and persistent mental health disabilities. The program serves youth that are exiting children’s services, receiving adult mental health services in need of age-appropriate support, and those with no mental health service history or those who have only accessed county crisis programs.

TAYP provides intensive case management, outreach and engagement, medication support, as well as, mental health counseling, peer groups, and supportive services to assist TAY transition to adult life. The goals for TAY are designed to improve personal wellness and community life functioning, care and prevention, as well as, daily living skills that promote healthy living and independence.

Stars TAY program is supported by mental health clinicians, case managers, peer mentors, and a psychiatrist, whom all collaborate for TAY treatment. The referral process is generally made by family members, county crisis programs, counselors, and foster family agencies. Clinicians may use the TAT referral form to make referral to the Transition Age Team (TAT), which meets weekly to review cases. Referrals through Alameda County Behavioral Health Services are also available.

School Success Program
The School success Program, which is funded through the Early and Periodic Screening, Diagnosis and Treatment program, provides supportive services and partners with students, families, school staff, and other community partners to help students succeed socially and academically. Supportive services, which take place on campus and in the community, may include:

• Individual and/or Family Therapeutic Services to focus on matters affecting school success and social or family relationships, such as trauma and transition.

• Behavioral Support to help students strengthen skills (communication, decision making, problem solving, coping) to enhance classroom and social success. This support can take place during class session, if needed.

• Groups to help students strengthen social and life skills and to help families strengthen relationships and support systems.

• Case Management to link students with community services or other additional supports related to individual and family needs.

Foster Care Youth Program
Our specialized therapeutic services are provided to youth who have experienced serious trauma and loss. Employing a strength-based, relational, and team approach, we offer a range of therapeutic and support services to foster care youth ranging in age from 3 to 21 years. Our foster care program is community based, provided to Alameda County youth placed within 90 miles of our San Leandro site. We specifically address issues relating to mental health, such as emotional disregulation, depression and anxiety, coping with trauma, improving interpersonal relationships, and identifying and utilizing healthy coping skills. We also assist with building and maintaining independent living skills, including linking to housing, and providing support for a young person’s educational and vocational goals.

Our services are trauma-informed, culturally aware, LGBT inclusive, and individualized to each client. We are also a Katie A provider and can offer Child and Family team meetings, as well as extra behavioral and emotional support for identified subclass members. To qualify for services, a youth must be a dependent of Alameda County or have been a foster youth previously, or in a family maintenance type program linked to Alameda County, as well as have active Medi-cal. TAY Resource Center
Stars opened this on-site in 2010 to facilitate access to resources and supports. It includes an educational and vocational component to assist participants with school or vocational engagement upon exiting.

In-Home Outreach Teams (IHOT)
A program offered by Stars Community Services that supports young adults ages 18-24 with challenges related to untreated mental illness and/or addiction. IHOT helps young people link successfully to responsive and recovery-oriented care within our community.
